I am in the process of updating my fungicide workbook for the upcoming season.  I have made a few changes on the template for fungicide scheduling.  Since many people want to have an Excel (or other spreadsheet) version of it.  I created a link of a template (just a form for printing), as well as a Google Document (You can download the file from this page).
